LETTER OF ACKNOWLEDGEMENT

This is to acknowledge that I, a freelance tutor / teacher of UNHOOP PHILIPPINES, INC., have read and understood the following conditions (please read and tick the box for ALL):

In case I can’t submit it on time, I’m aware that I need to contact TMD right away and give them an update. In the event that I have not yet submitted my SD by the time I get my first pay, I opt to (please sign before the option you’ve chosen):

Agreement Option
Option A: Have 10% EWT withheld from my salary and get my salary. I understand that this will be the case for all of my salary from Unhoop Philippines, Inc. until the end of the calendar year.
Option B: Get my salary after I’ve submitted my SD and have the appropriate EWT rate applied.

I hereby affix my signature below as proof of my agreement to the statements above.
